Hygiene Driven Health

In the last decade, there's been a huge paradigm shift in almost every aspect of periodontal diseases. Modern research has repeatedly demonstrated that the old traditional concepts of etiology and treatment were fundamentally flawed. The old simplistic concept that calculus caused periodontal disease has been replaced with a more scientific and accurate concept wherein specific bacterial infections trigger destructive immunological reactions.

Periodontal Disease

Clinical manifestations of gum disease are only the macroscopic effects of a microscopic infection. Pockets and bleeding are not the cause of disease, they are the result of disease. Determining who needs treatment is important. It is also important to know whether the treatment is effective and if the patient is able to maintain the results without having to wait for months or years for additional attachment loss to occur.

Gum disease is typically caused by a mixed infection of predominantly anerobic micro-organisms. Most of the tissue damage is caused by an immunological response that progressively destroys the supporting tissues.Conventional periodontal therapy often fails because the typical scrapodontic approach fails to eliminate the pathogenic microflora and traditional home care regimens are seldom designed to prevent reinfection. Anti-infective Periodontal Therapy exploits diagnosing and treating periodontal diseases as infections.

Clinical evaluation

Conventional clinical examinations reveal past damage and are vastly improved by laboratory and chairside assays such as phase microscopy that can detect the microbial risk factors that precede periodontal disease and verify their absence after therapy. Traditional debridement is enhanced by the incorporation of antimicrobial agents to help eradicate disease associated microorganisms. Anti-infective home care regimens are also designed to prevent reinfection.

Systemic Disease

Presently the major rationale for the treatment of periodontal disease is to prevent its progression and preserve the dentition and facial skeleton. There is increasing evidence implicating gum disease as a factor in systemic conditions such as sub-acute bacterial endocarditis, diabetes, coronary artery disease and strokes.

It's an exciting world of new paradigms for dentistry. Look to our hygienists for advice and innovative solutions to implement state-of-the-art dental care for the 21st century!
